On Arrival at the Port of Entry 1. The AQCSs Officer will carry out physical examination of the dog(s) as per the prescribed format (Annexure 1.3) at the port of entry. 2. In case the dog(s) is/are found sound and healthy (as per the Annexure 1.3), final NOC (Annexure 1.5) will be issued at the port of entry itself.
This means that for dogs that have been only in dog rabies-free or low-risk countries for the last 6 months, the one form you will need is the CDC Dog Import Form, which should take a few minutes to complete.

To prevent rabies from entering the state, the current law requires that dogs, cats and carnivores complete either the 120-day or 5 Day Or Less rabies quarantine. The law also requires that entire cost of the rabies quarantine program be paid by the users of the quarantine facility.
FAVN Blood Test: Rabies antibody test showing sufficient immunity. Must be done at an approved lab at least 30 days before arrival. Waiting Period: 30-day minimum from date of passing FAVN test before arrival in Hawaii.
