2. Begin by filling out the top section, including your Federal Employer Identification Number (FEIN), CRS Identification Number, and the quarter ending date formatted as mm/dd/yyyy. Ensure you select whether this is an original, amended, or supplemental report.
3. In the employee details section, enter each employee's Social Security Number and name in the specified format: Last name, First name Middle initial.
4. For each employee, input their gross wages for the quarter in column 3, state income tax withheld in column 4, and any Workers' Compensation fees due in column 5.
5. At the bottom of the form, calculate and enter the totals for columns 3, 4, and 5. If using supplemental schedules, ensure to include those totals as well.
6. Finally, sign and date the report. Include your title, email address, and phone number before submitting it online or via mail.
