4. In Section C, document new associations by listing individuals who have recently joined your provision. Include their title, first name, surname, position, and date started.
5. Finally, complete Section D for new additional contacts. Provide their details and indicate whether they should have access to complete the online self-evaluation form.
6. Review all entries for accuracy before signing and dating the form at the end.
