Prior Authorization: Your doctors office will submit a request to your insurance company to get approval for you to start treatment with ENTYVIO. Keep in mind that the authorization process may take several weeks. Your doctors office will be in touch if they need additional information.

Is informed consent required for treatment?

(c) No medical treatment may be administered to a client without informed consent except in an emergency situation as defined by Section 853 or circumstances otherwise authorized by law.

For UC patients continuing on subcutaneous maintenance therapy, administer ENTYVIO by intravenous infusion at 0 and 2 weeks before transitioning to subcutaneous ENTYVIO.

Some Entyvio side effects may occur during the injection. Tell your caregiver if you feel dizzy, nauseated, light-headed, itchy, sweaty, or have a headache, chest tightness, back pain, trouble breathing, or swelling in your face. Vedolizumab may cause a serious brain infection that can lead to disability or death.

Who is eligible for ENTYVIO connect?

Patients must be currently receiving ENTYVIO therapy and experiencing a gap in or loss of commercial coverage. The Bridge Program is not available to patients who are eligible for federal or state healthcare programs (Medicare, Medicaid, TRICARE, etc.).
