In section 2, provide your identification number and employer identification number. If applicable, include your social security number.
Specify your type of business in section 3, choosing from options like Corporation, Partnership, or LLC/LLP.
Fill out sections 4 through 9 with your legal name, USDOT number, DBA name (if different), business phone number, and mailing address.
Indicate whether you will be traveling outside New York State in section 10 and answer questions regarding highway use tax registration and previous IFTA licenses in sections 11 and 13.
Complete the decal order section by entering the number of IFTA vehicles and calculating the total due in sections 15 through 17.
Finally, certify your application by signing at the bottom of the form and providing your contact information.
