2. Begin by entering your Student Name exactly as you want it to appear on your diploma. Fill in your RSC ID# and any previous or maiden names if applicable.
3. Indicate whether you plan to participate in graduation exercises this spring by selecting 'Yes' or 'No'. If you require accommodations, specify the type needed.
4. Provide your mailing address where your diploma will be sent, including street, city, state, and zip code. Don’t forget to include your home telephone number and an alternate number.
5. If you have a previous degree from RSC, fill in the degree and year earned. Then indicate when you expect to complete your current degree by selecting Fall, Spring, or Summer along with the year.
6. Specify what degree you are completing and indicate if you are receiving more than one degree. If so, choose which division you'd prefer to walk with during graduation.
7. Confirm if you have a SOCNAV agreement on file. Finally, sign and date the form at the bottom before submitting it.
