To qualify for financial assistance, all of the following conditions must be met: The patient must be uninsured or, in certain circumstances, have limited insurance coverage. The patient must be unable to access other programs that would cover medical expenses.
Hawaiis Medicaid/CHIP is called Med-QUEST (QUEST stands for Quality care, Universal access, Efficient utilization, Stabilizing costs, and Transforming the way health care is provided to recipients).
You can complete your renewal in any of the following ways: Complete your renewal online at medical.mybenefits.hawaii.gov. Call us to complete your renewal over the phone. Complete, sign, and return this form to us by mail, email or fax. Visit one of our offices to complete your renewal in person.
The applicant income limit is equivalent to 100% of HIs Federal Poverty Level (FPL), which increases annually in January. Specific to Med-QUEST, the program income limits increase later in the year. Effective Feb. 2025, an applicant, regardless of marital status can have a monthly income up to $1,500.
