The Pain Disability Quality-Of-Life Questionnaire-Spine (PDQQ-S) is a short, six-question composite questionnaire that explores three domains of the pain experience - pain quality, disability, and life satisfaction/quality.
The items of the questionnaire are assessed on a 010 numeric rating scale in which 0 means no disability and 10 is maximum disability. The sum of the seven items equals the total score of the PDI, which ranges from 0 to 70, with higher scores reflecting higher interference of pain with daily activities.
Neck Pain and Disability Scale (NPAD) [1, 2] Item scores range from 0 to 5, and the total score is the sum of the item scores [possible range 0 (no pain)100 (maximal pain)]. A valid NPAD score can be generated if no more than 15% of the items are missing [10].
The average VA disability rating for neck pain generally ranges between 10 percent and 40 percent, depending on the severity of the injury. Neck pain is usually designated in VA claims as cervical spine injury and may be evaluated along with or in addition to claims for back pain.
Each item is scored out of five (with the no disability response given a score of 0) giving a total score for the questionnaire out of 50. Higher scores represent greater disability. The result can be expressed as a percentage (score out of 100) by doubling the total score.

Item scores range from 0 to 5, and the total score is a total of the item scores (possible range 0 (no pain) 100 (maximal pain)). The NPAD score is well validated if no more than 15% of the items are missing. Completion of the NPAD usually requires less than 5 minutes.
The Pain Disability Questionnaire (PDQ) aims to measure disability. It is derived from clinical research that indicates that biopsychosocial factors interact mutually during the development of pain and disability.
The NDI consists of ten questions in the following domains: Pain Intensity, Personal Care, Lifting, Reading, Headaches, Concentration, Work, Driving, Sleeping, and Recreation. Scoring: Each question contains six answer choices, scored from 0 (no disability) to 5 (complete disability).
