4. Specify the expected start and end dates for the exchange visitor's stay. Choose the appropriate exchange visitor category from Professor, Research Scholar, or Short-Term Scholar.
5. Complete the Health Insurance Information by selecting who is providing health insurance and ensuring it meets minimum coverage requirements.
6. Fill out Financial Support Information by selecting all applicable sources of funding and listing total amounts from each source.
7. Provide J-1 Exchange Visitor Information including personal details such as name, date of birth, citizenship, and current immigration status.
8. If applicable, enter information for up to three J-2 dependents. For additional dependents, attach a separate document.
9. Review all sections for accuracy before electronically signing and saving the form to email it to OISSS at oisss@utoledo.edu.
