NC Medicaid Direct is North Carolinas health care program for NC Medicaid beneficiaries who are not enrolled in NC Medicaid Managed Care. It includes care management by Community Care of North Carolina (CCNC), the primary care case management entity for physical health services.

Broadly, there are four major eligibility groups covered by most states: children, adults with disabilities, aged adults, and nondisabled adults.
NC Health Choice is a free or reduced cost health insurance program for uninsured children birth through age 18. Who is it for? Children who have family incomes that are too high for Medicaid coverage, but equal to or less than 200% of the federal poverty guidelines.
NC Health Choice will no longer be an offered program, beginning April 1, 2023.
