How It Works. You order an aspect of your student record online, sign an online release authorizing your high school to release your records to your designated recipients, and pay document handling and shipping fees online.
Complete and submit NYSED Transcript Request Form to OCUEINFO@nysed.gov. The request must include a signature from the former student granting permission to release the transcript.
Download and fill out the Student Records Request Form. Use the School Finder tool to navigate to your schools website for contact information. Email your school and attach a copy of your photo ID (for example, a passport, state-issued photo ID, or an IDNYC ID) and the filled-out Student Record Request Form.
Please fill out a school Transcript Request Form and fax it directly to the school. You can find school fax numbers on the school listings pages. If your high school has merged into another school, the new school often has copies of transcripts and you can send your request there.
