The PT committee is composed of actively partici- pating physicians, other prescribers, pharmacists, nurses, administrators, quality-improvement managers, and other health care professionals and staff who participate in the medication-use process.
The PT committee ensures that mechanisms are in place to communicate with health care professionals, patients, and pay- ers about all aspects of the formulary system, including changes made to the formulary or policies and how formulary system decisions are made.
The Pharmacy and Therapeutics Committee evaluates, analyzes and recommends treatment protocols and procedures for the timely use of and access to both formulary and non-formulary drug products at least annually.
(b) The PT Committee shall consist of the Executive Medical Director, and six members appointed by the Administrative Director. (1) The Executive Medical Director, or his or her designee, shall serve as chairperson of the PT Committee.
(2) The Systemwide PT Committee shall report to the statewide Quality Management Committee (QMC).

The basic objectives of a PT committee are to specify drugs of choice and alternatives, based on safety and efficacy; to minimize therapeutic redundancies; and to maximize cost-effectiveness. Procedures necessary to strengthen the role of PT committees and improve their decision-making processes are discussed.
(a) The PT Committee shall meet when deemed necessary by the Executive Medical Director, but no less frequently than quarterly. (b) PT Committee meetings shall be conducted in ance with the Bagley-Keene Open Meeting Act, California Government Code sections 11120 through 11132.
Expired medications are ineffective, potentially unsafe, and cannot be dispensed. On a monthly basis, pharmacy personnel must check the items in the pharmacys inventory to ensure that any expired products are removed from stock. Pharmacies also receive notification of recalls from manufacturers whenever they occur.
