1. Click ‘Get Form’ to open the permit corrections form in the editor.
2. Fill in the date and P.R. number at the top of the form. Select the permit type by circling either 'Commercial' or 'Residential'.
3. In the first column, indicate where additional or corrected documents should be delivered for review by selecting from the review groups listed.
4. Complete the signature section, ensuring you print your name, provide a phone number, and confirm your authorization to submit corrections on behalf of the property owner or contractor.
5. On the reverse side, specify any items being added or replaced in your plans. Ensure to include quantities for each item submitted.
