2. Begin by entering your personal information at the top of the form. Fill in your name, student ID, email address, and phone number accurately.
3. Indicate your major, minor, or certificate by circling the appropriate option. If applicable, include any concentration or emphasis.
4. Answer whether you have applied for graduation by selecting 'Yes' or 'No' and provide the date of application.
5. In the Catalog Eligibility and Declaration section, state the catalog year and circle your chosen major/minor/certificate catalog. Check the boxes that apply to your enrollment status.
6. If you are requesting an exception to the catalog declaration policy, ensure you sign and date where indicated. Obtain signatures from your advisor and dean as required.
7. Once completed, return the form to the Office of the Registrar as instructed at the bottom of the document.
