What counts as proof of immunization?
Photocopies of immunization records from a licensed healthcare provider or clinic, or from a California K-12 school uploaded to My Health Portal. All records must show the vaccine type, dose, and date of each shot.
Does Harvard require vaccination?
The COVID-19 vaccine is strongly encouraged, but not required, for all students, faculty, staff, and researchers.
Do transcripts have immunization records?
If your doctor has proof of your immunization history, this may be your preferred option. If you went to high school in the United States, it is very likely your records are on your high school transcript. If you already have these records, check and see if your immunization history is there!
Does your school have your immunization records?
Most schools keep vaccination records on file. Schools may keep records for a year or two after a student leaves or graduates. Contact your childs school for more information.
Do you have to have immunization records for college?
Students entering any college or university must show proof of immunity against tetanus, diphtheria, pertussis, MMR, hepatitis B, and varicella. Students in a dormitory or comparable congregate living arrangements must have vaccination against meningococcal.
