1. Click ‘Get Form’ to open the 1652 form template in the editor.
2. Begin with Part A by entering your agency name, date of request, and agency point of contact details. Ensure all information is accurate for processing.

6. For endorsement, have the Department/Agency head sign in Box 13 and enter their name in Box 12. This certifies that the candidate meets necessary qualifications.
7. If applicable, complete Part B for White House Liaison endorsement before submitting your form to OPM via fax as instructed.
