Follow-up Steps Required by Law: 259.11(a) states that name change applicants must file a certified copy of the order with the County Recorder of each county where they, their spouse/domestic partner, or minor children claim an interest in land.
A certified copy of the courts order to change your name can be requested from the court that issued the order. The Social Security office is one place that asks for official proof of a name change. The Department of Motor Vehicles is another. This is so you can get a new Social Security card and state ID.
After the Order is signed, you can obtain certified copies of the Order from the Court Administrators office for a fee of $14 each. Certified copies of this order will be needed to change your personal records (see the document entitled Notice to Persons Who Have Had Their Name Changed for more information).
This service cost $99.95 for an Adult or Minor name change. Our fee includes the preparation of the forms and delivery of the forms in a data format. Paper copies of the forms can be mailed to you (upon special request) for an additional charge.
How long will the entire application for change of name take? Name change actions can take anywhere from a day to six (6) months (sometimes even longer).
