If you go out of network, the VSP individual plan limits the coverage members can receive for out-of-pocket costs. If you have this plan and choose to go out of network, you can expect to pay most of the full cost per visit.
If you use a Costco, Wal-Mart or other non-network provider, you will need to pay the entire bill at the time services are provided and submit a completed claim form to EyeMed Vision Care. For services at all Costco and Wal-Mart optical locations, youll receive reimbursements equivalent to in-network benefits.
However, if you see an out-of-network provider AND you have out-of-network benefits as part of your plan, youll need to pay at the time of service and submit a claim form online for reimbursement. You will need to upload an itemized paid receipt with your name included.
