The clerk (i) may, on his own authority, correct marriage records established in his office by amending the same upon application under oath and submission of evidence deemed by the clerk to be adequate and sufficient and (ii) shall correct such records upon order of the court in which the marriage record was
Corrections are made by completing an Affidavit to Amend a Marriage Record (VS 24C). This form becomes a part of the original recordtype or print clearly in black ink only. Your certified copy will include a copy of the original certificate with a copy of this form attached.
To correct/amend the affidavit, license, and certificate of marriage record by mail, please complete and sign the Affidavit for Correction of Marriage form (DOH-1827). If you are unable to download the application, please call (855)-322-1022.
The following is a list of documents to include: Original affidavit signed by the person requesting the correction. A $15 check or money order made payable to IDPH. A copy of a non-expired, government issued photo ID of the person requesting the correction. Documentation required to complete the correction requested.

Both spouses are required to appear in person at any of our office locations to have the amended marriage license issued. You are required to provide documentation to support any information that is changed (If spelling of a name is amended you will need to provide a birth certificate as proof of spelling).
