2. Begin by entering your Court File Number, Judicial Centre, and both parties' complete names at the top of the form. Ensure your address and phone number are also included.
3. Carefully read through each item listed in the form. Check off the items you wish to request from the other party, keeping in mind that certain items cannot be requested after a final court decision.
4. Once completed, make two copies of the filled form. Bring all three copies to the Court of Queen’s Bench Chambers counter for filing along with your Disclosure Statement.
5. Serve one copy of both documents to the other party either by hand or recorded mail. It’s beneficial to provide them with a copy of the 'Providing Financial Disclosure' package as well.
6. If you receive the necessary information, contact the Chambers Clerk to cancel your court date. If not, prepare an Affidavit detailing what documents were not provided and what remedy you seek.
