6. Complete Part 3 by certifying your application in front of a witness. Choose the correct certification option based on your marital status.
7. If applicable, have your spouse complete Part 4 for their consent, ensuring they sign in front of a witness as well.
8. Once all parts are completed, submit the application directly to your financial institution and keep a copy for your records.
