4. Display the amusement machine certificate in a conspicuous place at the location where the machines are being used.
5. If your certificate is stolen or vandalized, send a written request for a replacement to the provided address, signed by the machine operator.
6. To operate more machines than you currently have certificates for, complete another Application for Amusement Machine Certificate (Form DR-18) and pay an additional $30 per machine.
7. If moving machines within the same county, contact the Department to correct the location on your certificate. For moves to another county, ensure you are registered for sales tax there first.
8. Remember that your amusement machine certificate expires on June 30th each year; renew it before this date with a fee of $30 per machine.
