Begin by entering your name and Social Security number (or employer identification number) as they appear on your tax return. If applicable, include your spouse’s name and SSN.
Complete columns A through G for each item or service costing $25,000 or more. Start with column A by entering the delivery date of the item or service.
In column B, provide a detailed description of the purchased item or service. Ensure clarity for accurate processing.
For column C, input the seller’s name and address. If purchased online, include the seller's website address.
Column D requires you to enter both the delivery address and the location of use if different from where it was delivered.
In column E, record the purchase price as indicated in your previous worksheets related to sales tax.
If applicable, enter any tax paid to another jurisdiction in column F. Finally, total up the amounts in column G for line 2 to ensure it matches your reported sales tax.
