2. Begin by entering your business name, address, and employer account number at the top of the form. Ensure that you also include the quarter ended date and year.
3. Verify your Federal Employer Identification Number (FEIN). If it’s incorrect, cross it out and enter the correct number. Add any additional FEINs in the provided boxes.
4. If no wages were paid this quarter, check Item A. If you are out of business or have no employees, check Item B and provide the out-of-business date in Item B1.
5. For total subject wages paid this quarter, fill in Item C with the total amount. Proceed to calculate Unemployment Insurance (UI) contributions in Items D1-D3 based on your taxable wages.
6. Complete Items E and F for Employment Training Tax (ETT) and State Disability Insurance (SDI), respectively, using their respective rates and taxable wages.
7. Enter California Personal Income Tax withheld in Item G. Then calculate the subtotal in Item H by adding Items D3, E2, F3, and G.
8. In Item I, list all contributions paid for the quarter. Finally, calculate total taxes due or overpaid in Item J by subtracting Item I from Item H.
9. Sign and date the form in Item K before submitting it to ensure compliance.
