  2. Begin by filling in the student’s name, grade, date, and interviewer details at the top of the form. This sets a clear context for your assessment.
  3. In the 'Student Strengths' section, identify at least three strengths or contributions from the student. Consider academic, social/recreational, and other relevant areas.
  4. Move to the 'Routines Analysis' section. Document where, when, and with whom problem behaviors are most likely to occur. Use a scale from low to high to assess likelihood and specify current interventions.
  5. List prioritized routines for behavior support based on ratings of 5 or 6. Ensure you only combine routines with significant similarities in activities and problem behaviors.
  6. For each identified routine, rank order the top priority problem behaviors and describe them in observable terms. Include frequency and duration of these behaviors.
  7. Complete FACTS-Part B by selecting one prioritized routine for detailed assessment. Rank antecedents, consequences, and setting events that may influence behavior.
