To receive a copy of your loved ones health records as a Personal Representative, you typically must ask for it in writing. In some cases, the hospital or doctors office may have a form to request the records. Even with the legal right, however, this can prove to be a challenging process.
How you make your request will depend on your providers processes. You may be able to request your record through your providers patient portal. You may have to fill out a form called a health or medical record release form, or request for accesssend an email, or mail or fax a letter to your provider.
To obtain a copy of your hospital records you will need to apply in writing to the hospitals records manager at the hospital where you received your treatment.
This can be quicker than getting access to your full historic record. The surgery will consider your request and make sure there are no issues with making the information available to you (for example, if it could put you or someone else at risk of harm). This can take a few weeks.
South Carolina: For paper and electronic copies, 83 per page for the first 30 pages, 63 per page for each additional page, plus a $32.06 clerical fee. For paper copies, altogether these fees cannot exceed $256.58, and for electronic records, these combined fees cannot exceed $192.44.
