In Section Two, indicate the semester you are requesting for readmission by checking the appropriate box and filling in the year. If applicable, provide your academic major when you last attended SRU and state your reason for withdrawal.
If you left SRU with a GPA below 2.0, complete Section Three by drafting a letter that outlines your academic goals, steps taken for success, and activities since withdrawal. Attach this letter to your application.
Finally, review all entered information for accuracy. Print the completed form, sign it, date it, and forward it as directed to Academic Services along with any required transcripts.
