Begin by filling out the Adult Volunteer or Parent/Guardian Information section. Enter your name, relationship to the youth, address (including city and zip), phone number, email address, and your school or company.
If there is a second adult volunteer or guardian, repeat step 2 for their information.
Next, move to the Youth Volunteer Information section. For each youth under 18, provide their name, birthday, school, cell phone number (if any), email address, and any relevant skills they possess.
Review the consent statement regarding participation in OSF activities and media release. Ensure you understand and agree before signing.
Finally, sign and date the form at the bottom as an Adult Volunteer or Parent/Guardian.
