Begin by entering the Corporate Name in Article 1. Ensure that it is clear and accurately reflects your organization.
In Article 2, provide the Name and Address of your Registered Agent. Fill in the First, Middle, and Last Name, followed by the complete address including Suite number if applicable.
For Article 3, specify the number of Directors (minimum three) and list their Names and Addresses clearly.
In Article 4, outline the purpose(s) for which your Corporation is organized. If applicable, check 'Yes' or 'No' for questions regarding Condominium Associations, Cooperative Housing Corporations, or Homeowner's Associations.
Complete Article 5 with any additional provisions. Use extra sheets if necessary.
Finally, in Article 6, have all incorporators sign and print their names along with their addresses. Remember to date the document appropriately.
