  5. Fill out your personal information. Ensure all required fields are completed, especially contact and licensure information, then click 'Continue'.
  6. Review any continuing NCC online modules taken since January 1, 2009. Manually enter any hard copy modules if necessary.
  7. List non-NCC related continuing education activities. Use appropriate specialty codes and click 'ADD ACTIVITY' for each entry.
  8. Complete payment details based on your selected options and click 'Continue'.
  9. Review all entered data for accuracy. Make changes as needed before clicking 'Send application' to submit.
  10. After submission, you will receive a confirmation email. Keep this for your records and complete any additional surveys as prompted.
