Request a Record You can search for your or your childs immunization record by visiting My Vaccine Record. To learn how to access records, click on that sites Accessing Records tab. If a childs health care provider has listed you as the parent/guardian in the CIR, you may access their record.
Submit a copy of your immunization records from a previous school or your health care provider. (Must be on official letterhead; copy from doctor must be signed and include office address and phone. Many medical practices or previous schools have an online portal you can use to print an official copy to send to UB.)
What address should I send my official transcript to? Digital/PDF transcripts emailed to BMCC/CUNY by the institution or school they are coming from are preferred. To send transcripts to CUNY/UAPC, please email official transcript directly by the institute to: admissions@cuny.edu or transcripts@cuny.edu.
