The affidavit will not be accepted by the Treasurers office unless all areas 1 to 7 are fully completed. Four copies will print. Mobile homes require a separate affidavit for change of ownership.

Corporations, partnerships, sole proprietorships, joint ventures, nonprofit organizations, limited liability organizations, etc., conducting business in this state are all subject to these taxes, even if they do not have an office here.
