Innovation Awards 2013-2025

1. Click ‘Get Form’ to open the Innovation Awards 2013 application form in the editor.
2. Begin by filling in your personal details, including the Name of Applicant(s), Position, and whether it's a Team or Individual Entry. Ensure you provide a Primary Contact for Team Entries.
3. Next, complete the Service and Employer fields, followed by your Email Address, Work Phone, and Work Address. This information is crucial for communication regarding your application.
4. Prepare a brief abstract of your innovation. Remember, it should be one page only; otherwise, your application may be returned. You can include two additional pages of information if necessary.
5. Review the Guidelines for Applicants carefully to ensure all required content is included in your abstract. Pay attention to objectives, process, results, evidence, and value as outlined.
6. Once completed, use the Tab key to navigate through the boxes efficiently. Save your document and email it along with your abstract as attachments to colleen.lambourne@bopdhb.govt.nz.
