  5. Provide a detailed explanation of why additional funding is needed to assist the individual in finding or keeping a job.
  6. Describe how the current job or job search aligns with the individual's skills, interests, and preferences.
  7. List what services will be provided with the requested additional funding and estimate how many units of each service are needed.
  8. Explain why current services included in the ISP cannot be discontinued to provide funding for employment supports.
  9. Detail any assistive technology, accommodations, or supports utilized to help the individual succeed in their job.
  10. Indicate if other non-DDD funded services are available and provide an explanation.
  11. If applicable, check 'YES' for additional assessments completed and provide details including date and who completed it.
  12. Sign off on the form by completing your signature and relationship to the individual before submitting it for review.
