How To: Air Force veterans who separated or retired on or after Oct. 1, 2004, may request their records in two ways: Register for a free Premium account on the Department of Veterans Affairs eBenefits site. E-mail, mail or fax a signed SF-180 to AFPCs military personnel records section.
Submit DD Form 149, Application for Correction of Military Records to the relevant service branch. (You can either view and download the form, or right-click the form and select Save link as or otherwise save the file and then open it.)

If your name change request DOES NOT meet any of the exact criteria above, you must submit a DD Form 149, Application for Correction of Military Record under the Provision of Title 10, U.S. Code, Section 1552, through the AFRBA website at or by mail to the address on the back of the form
Application for Correction to Military Records must be submitted to the CABCMR three years following the alleged error or injustice date. Failure to file within the three year time limit prescribed will only be waived when the board finds it to be in the best interest of justice to do so.
