Complete the 'Quote for the cost of procedure' section by entering accurate estimates for surgeon fees, hospital accommodation, anaesthetist fees, and other related costs.
Finally, review the 'Declaration and authorisation' section. Ensure all required signatures are provided before submitting your completed form.

In your instance of Accuro using the menu icon find the Form Editor Page 2 In Form Editor select Tools and then Publish/Download Page 3 In Form Publisher select Download Forms tab. You can filter the Publisher (eg. wejerrett) and then search for the form you are looking for.
