3. Once you have completed 'Section A', reach out to the International Student Advisor at your current institution. They will need to fill out 'Section B' of the form, which includes important details such as the last date of attendance and SEVIS ID number.
4. After both sections are completed, gather photocopies of required documents: front and back of your I-94 card, visa stamp in your passport, EAD cards (if applicable), and all pages of any previously issued I-20s.
5. Submit the completed form along with all photocopies to the UCSF International Students & Scholars Office for processing.
