In the 'Billing Information' section, provide your affiliation/agency name, billing address, account/PO number, and MLML grant number (this is required).
Fill in details for the Chief Scientist including their name and phone number. Also indicate the number of people in your party.
Complete the 'Float Plan' section by detailing your area of operation, type of work being conducted, and any equipment or technician needed.
If applicable, fill out the CA Fish and Game Permit information including your name and permit number. Remember to check if a 24-hour notice of collection is required.
Finally, ensure that both signatures are obtained: one from the Chief Scientist and another from the MLML Director or Marine Superintendent along with their respective dates.
