The Form MCSA-‐5850 is a secure, online form that is only available through your Na(onal Registry account. You must submit the form through your Na(onal Registry account. FMCSA will not accept submissions by any other means. For example, you may not fax the report or upload scanned forms or spreadsheets.
The FMCSA requires all interstate carriers to file a MCS-150 or MCS-150B, if a Safety permit is required, every 24 months based on the last two digits of the US DOT number. The last digit of the number dictates the month and the second to last digit dictates the year.
Whats the difference between MCS-150 and MCSA-5889? MCS-150 is for USDOT updates and MCSA is for Motor Carrier (Operating Authority) updates for Name Change, Address Change and Reinstatement.
