Click ‘Get Form’ to open the Application Form - MyWCER in the editor.
Begin by filling out the Applicant Information section. Enter your Name, Title, WCER Project, Campus Address, Campus Phone, and Email accurately.
Proceed to the Activity Information section. Provide the Title of Activity, Beginning Date, Ending Date, and a Brief abstract (50 words or less) summarizing your activity.
In the CHECKLIST section, ensure you check off each required item. This includes the Proposal Cover Page, Professional Development Grant Proposal, Budget worksheet and supporting documentation, documented information on the activity, and your Brief Resume/CV.
Complete the Supervisor section by entering their Name and Signature. Indicate if funding is available from your project that could supplement WCER funding.
Fill in the Total Cost of Activity and Total WCER PD Grant Request fields before entering the Date of Submission.
Finally, submit an electronic copy of your application along with any supporting materials to Mary Hegge at mhegge@wisc.edu.
