3. Fill in the 'Name' fields: print the current resident's name in the first column and the new roommate's name in the second column.
4. Input each person's 'ID Number' and 'Birth Date', ensuring accuracy for identification purposes.
5. In the 'Eligibility Status' section, circle the appropriate status for both individuals (Student, Faculty, Academic Staff, Post Doc).
6. Provide the 'Department Name' and 'Department Phone' for each person involved.
7. Specify the 'Date expected to move in' for the new roommate, which will be used for key issuance.
8. Agree on a date when both roommates will be legally liable for rent; this must be after any previous roommate's liability ends.
9. Both roommates must sign and date at the bottom of the form to finalize their agreement.
