consent. The TG21 form is primarily a consent form which has exploded into a medical document. It only need be an A5 slip. Name, rank, unit. Event, location, date.
TG22 is the ADULT consent form and is just for staff. TG23 is the Health Declaration form and is for cadets or staff who require them due to some declared medical condition.
Cadets under 18 will need parents/carers to complete a form TG21 for every activity. If a cadet has a medical issue, additionally a TG23 form will need to be completed for every activity. For flying and gliding activities, the TG21 (and TG23 if applicable) and the AV-MED1 forms are required.
A new calibration protocol, developed by the AAPM Task Group 51 (TG‐51) to replace the TG‐21 protocol, is based on an absorbed‐dose to water standard and calibration factor ( N D , w ) , while the TG‐21 protocol is based on an exposure (or air‐kerma) standard and calibration factor ( N x ) .
