ADIME stands for Assessment, Diagnosis, Intervention, and Monitoring and Evaluation, which form the four main sections of the ADIME notes.
The ADIME format is an excellent tool to help organize the flow of your nutrition appointments and document your process. The ADIME format helps to ensure similar processes are followed by each RDN. This is done to allow better continuity of care.
What should be included in ADIME note? This will differ for every client, but some details should always be included. These are the patient information such as their name and birthdate, your signature, the date of the session, and the reason for their referral to you.
The Nutrition Care Process (NCP) involves 4 steps: assessment, diagnosis, intervention, and monitoring and evaluation. Dietitians can use the NCP as a consistent methodology to guide nutritional care. ADIME charting incorporates the steps of the NCP into a documentation style.
