California removed its personal and religious exemption option in 2015. Parent/guradian must complete an online educational module to receive a non-medical exemption. Connecticut removed its religious exemption option in 2021.
Required immunizations include MMR (measles, mumps and rubella), Tdap (tetanus, diphtheria, pertussis), Varicella (Chicken Pox), Hepatitis B, and meningitis vaccine.
Should students or student health offices complete the form? Students should complete the AAMC Standardized Immunization Form in conjunction with either the student health office at their medical school or with their primary care provider.
Students in public institutions of higher education must be immunized against Tdap, MMR, meningococcal disease and hepatitis B. A religious exemption is permitted. Public and private colleges that offer on-campus or group housing must provide information on meningococcal disease to each first-time student.
Submit your completed Student Immunization Record Form and/or existing vaccination records to the Wellness Center at wellness@gc.cuny.edu. A single PDF document attachment is preferred. You should submit your completed documents as early as possible to avoid potential delays in clearance for registration.
