Begin by filling out the Buyer Information section. Enter your company name, phone number, and address details including state, city, and ZIP code.
Provide the main contact's name and phone number, along with the buyer's name and their contact information. Include accounts payable details such as name, phone, and email.
Indicate the product or service you are applying for, years in business, number of employees, D&B number, and tax ID number. Specify if you are taxable or non-taxable.
In the Bank Information section, fill in your bank's name, branch details, contact person's name, phone/email, and address.
Complete the Trade References section by providing names and addresses of your trade references along with their contact information.
Finally, authorize the application by printing your name, title, signing it, and adding the date at the bottom of the form.
