Begin with Section I by entering the School District or Private School Information. Fill in the School Name, Contact Name, E-mail Address, Mailing Address, City, State, Zip Code, Physical Address, Phone Number, and Fax Number.
In Section II, indicate your interest in participating in the Alaska Vaccine Distribution Program by selecting 'Yes' or 'No'. If you are a current participant, provide your PIN number.
Proceed to Section III to provide details about your Electronic Health Record Software. Enter the name of the software, vendor, version, and contact information for both the vendor and school technical contact.
Review Section IV carefully as it contains important terms regarding usage rights and responsibilities. Ensure you understand these before proceeding.
Finally, complete Section V by signing and dating the form. Email the completed document to vactrak@alaska.gov.

Are vaccines mandatory in Alaska?
Per 4 AAC 06.055, all students must remain up-to-date with state required immunizations. All students enrolled in Alaska schools must be immunized against: Diphtheria, Tetanus, Pertussis (DTap/DT/Td/Tdap) Polio.
Additionally, most states offer some form of non-medical exemption, with only three states West Virginia, Mississippi, and California not allowing any exemptions to vaccination mandates for non-medical reasons [28].
