3. Choose your superannuation fund by placing an X in the appropriate box. If you select an APRA fund or RSA, complete items 2, 3, and 5. For a self-managed super fund (SMSF), fill out items 2, 4, and 5.
4. For item 3 or 4, enter the required details of your chosen fund including its name, address, and unique superannuation identifier (USI). Ensure all information is accurate to facilitate proper contributions.
5. Attach any necessary documentation confirming compliance with ATO regulations for SMSFs. Finally, sign and date the form before submitting it to your employer.
