  8. For each item listed regarding bedroom amenities (finished walls, floors, ceilings, heating, lighting, closet, bed with mattress, bedding), select 'Yes' or 'No'.

If you are unable to get your ROE, go to your nearest Service Canada Office or call them at 1-800-206-7218 or mail in this form. A Service Canada agent will tell you how to get your ROE or what else they will need to complete your application. Be sure to apply as soon as you dont have work.
Employers would first need to determine the total insurable earnings that the employee has received for each pay period. Thereafter, the employer would need to add all the insurable earnings together. This amount would be the total insurable earnings for the employee. Employers would need to enter it in Block 15B.
To obtain your ROE, you must contact Service Canada directly. You can access your ROE electronically by signing in or registering for a My Service Canada Account.
If 2 weeks go by and your employer has still not prepared your ROE, you should ask your employer again. You can write them a letter asking for your ROE using this tool. If you are unable to get your ROE, go to your nearest Service Canada Office or call them at 1-800-206-7218 or mail in this form.
Employers are obligated to issue an ROE to their employees when there is an interruption of earnings, such as a layoff, termination, or resignation.
