  2. Fill in the removal date, which indicates when the child was removed from their normal residence. This date is typically found in the Shelter Order.
  3. For Judicial Removal, ensure you have a Shelter/Initial Removal Order that includes 'contrary to welfare' and 'reasonable efforts to prevent removal' language. Attach the Shelter Petition detailing the removal situation.
  4. If applicable, complete the Voluntary Placement Agreement (CF-FSP 5004) and note its signed date. Ensure a court order is obtained within 180 days that also contains 'contrary to welfare' language.
  5. Gather SSI-related documentation, including an award letter from Social Security Administration if applicable, as this impacts eligibility.
  6. Provide AFDC/Eligibility-related documentation such as proof of citizenship status, social security card copy, and income and asset supporting documents.
  7. Finally, complete the Foster Care Eligibility section with input from a Revenue Maximization Specialist to determine IV-E eligibility status.
