2. Begin by selecting your membership type: Full Membership, Associate Membership, or Small Business. If you choose Small Business, provide your NAICS Code and revenue ceiling.
3. Fill in the Date, DOE Site/Facility Name, and Company Name as it should appear in the roster. Ensure that your Mail Address is complete with POB/Street, City, State, and Zip.
4. If your delivery address differs from your mail address, enter it in the designated fields. Include your Main Phone Number and details for the EFCOG Representative including their Full Name, Title, and contact information.
5. Indicate whether you are a registered lobbyist by checking 'Yes' or 'No'. Provide details for the Representative’s Executive Assistant if applicable.
6. Designate any personnel from your company who wish to participate in EFCOG working groups by filling out their names and contact information.
7. Complete the Member Profile section by summarizing what your company does and the DOE Programs you support. Indicate if your company qualifies as a small business.
8. Once all fields are filled out accurately, save your changes and return the completed form via email to bsb@longenecker-associates.com.
